Call Continuing Care Access at 780-496-1300 and ask for a home care assessment.
A Registered Nurse from AHS will visit your home, complete the assessment, and make recommendations.
Request StaySure Home Care as your preferred provider.
AHS will assign approved hours under the CDHCI program.
